New Patient Enrollment
Online Registration:
- Click here to go through our online enrollment process. This will enroll you in our online services including secure messaging with your physician, online refill and appointment requests, access to educational resources, and access to a summary of your medical records. When you put in this information it will also streamline the process of creating your chart in our electronic medical records. Once you register online a message will be sent to one of our Health Coordinators. They will then contact you to schedule your first visit with us. Click here for help with online registration.
- Click here to download and fill out the additional forms that you need to complete prior to your first visit.
- Download and fill out a Record Release Form. Please complete one form for each physician we need to request your records from. This helps us to gather your records from your previous physician so that you don't have to repeat a lot of your historical information.
- You will not be asked for any payment at the time of online registration. The annual fee is not charged until the date of your first visit.

Please remember to bring the following to your first appointment with us:
- Your insurance card
- Your photo identification
- Your Immunization record
- The completed Additional Forms from above
- A medical records release form completed for each physician that we need to request your records from
- A current list of your medications
- Payment of your GreenField Annual Fee. You may pay by cash, check, credit or debit card if you are paying your annual fee in full. You can also pay the fee monthly by payment plan by giving us a debit or credit card number.
