Phone Registration:

• Call our main number at 503-292-9560 and one of our staff members will get you registered and schedule your first visit with us.

• Download and fill out a Record Release Form. Please complete one form for each physician we need to request your records from. This helps us to gather your records from your previous physician so that you don't have to repeat a lot of your historical information.
  
  
• You will not be asked for any payment at the time of phone registration. The annual fee is not charged until the date of your first visit.

Please remember to bring the following to your first appointment with us:

• Your insurance card
• Your photo identification 
• Your Immunization record 
• The completed Additional Forms from above
• A medical records release form completed for each physician that we need to request your records from 
• A current list of your medications 
• Payment of your GreenField Annual Fee. You may pay by cash, check, credit or debit card if you are paying your annual fee in full. You can also pay the fee monthly by payment plan by giving us a debit or credit card number.
